Requirements

• Applicants must have exhausted other avenues of funding and have no prescription drug coverage.
• Information on status of applications to ADAP or Medicaid programs is required.
• Financial information is also requested, limits on earnings are not specified but are well above the Federal poverty guidelines.
• A doctor can enroll a patient for a 90 day period which can be extended with certification.
